Robotic Sleeve Gastrectomy

Gastric sleeve surgery is a restrictive type of procedure that permanently reduces the size of the stomach, thereby limiting food intake as well as the sensation of hunger. The smaller stomach size restricts the amount of food that can be eaten, so patients lose weight by eating fewer calories. This procedure also impacts the production of ghrelin.

Robotic Gastric Bypass

This is the most common type of bariatric surgery. During this type of procedure, part of the stomach is separated from the rest of the stomach and closed off with staples to create a smaller pouch, resulting in less calorie absorption. Patients also have a greater sense of satisfaction with less food. The surgery results in less production of ghrelin, a hormone that may be partly responsible for making you feel hungry.

Revisional and Conversion Procedures

A revision is a minimally invasive surgical option to modify or correct a previous bariatric surgery. There may be instances where some patients may need revision surgery for limited weight loss, weight regain, unresolved medical conditions, or complications. A conversion procedure replaces a previous bariatric surgery with a different procedure. Examples include replacing a Lap-band with a gastric sleeve or to a gastric bypass, or sleeve gastrectomy conversion to gastric bypass, especially in patients with insufficient weight loss, severe reflux, or gastroesophageal reflux disease (GERD).

Hypoabsorption is a term derived from "hypo," meaning less, and "absorption," the process by which the body takes in nutrients. Essentially, hypoabsorption indicates that the body absorbs fewer nutrients, particularly calories from fat.

When performed correctly, hypoabsorption can facilitate additional weight loss without leading to significant malnutrition. It's crucial to remember that "malnutrition" refers to poor nutritional status, and while hypoabsorption reduces the intake of certain nutrients, it does not automatically lead to malnourishment. Both with and without weight loss surgery, the risk of malnutrition and vitamin deficiencies exists. Adhering to the guidance provided by your bariatric team is essential to mitigate these risks.

Surgeries That Do Not Induce Hypoabsorption

Two common weight loss procedures—sleeve gastrectomy and gastric bypass—do not significantly result in hypoabsorption of calories.

  • Sleeve Gastrectomy involves removing a substantial portion of the stomach, creating a tube-like structure. This method effectively diminishes stomach size and alters hunger hormone levels.

  • Gastric Bypass not only reduces the stomach size but also reroutes a section of the small intestine. This surgery can affect hunger hormones and may have a minor impact on micronutrient absorption, but calorie absorption remains intact. It’s vital for patients to recognize that with either procedure, weight loss is primarily achieved through reduced calorie consumption. Failure to adhere to dietary recommendations may lead to insufficient weight loss or even weight gain.

Surgeries That Promote Hypoabsorption

Certain surgeries leverage hypoabsorption to enhance weight loss outcomes. The SADI-S (Single Anastomosis Duodenal-Ileal Bypass) and Duodenal Switch are more intricate procedures typically reserved for cases of severe obesity or challenging diabetes management. These surgeries combine a sleeve gastrectomy with the rerouting of the initial section of the small intestine, resulting in a smaller stomach, modifications to hunger hormones and metabolism, and a marked reduction in calorie absorption, particularly from fats. Consequently, these procedures often lead to greater weight loss compared to sleeve or gastric bypass surgeries.
